All existing patients should have been sent an invite to join the portal. The emailed invitation comes from the sender, Follow My Health. Please check junk mail or spam. If your link is not working, please call the office at 713-622-5480 for assistance.
If your account is active, you may sign in by going to www.followmyhealth.com and use the username and password you created. Follow My Health is also an app that you can download on your phone.
Patient Notice – Access to Health Records & Consumer Information

1. Requesting Your Medical Records
Patients may request copies of their medical records through one of the following methods:
• Preferred Method: Through the Follow My Health Patient Portal
• By Phone: 713-622-5480
Please include your full name, date of birth, and details regarding the records requested.
Requests for medical records will be processed within 15 business days as required by Texas law.

Consumers may file complaints regarding access to health records or other consumer matters under the Texas Health & Safety Code through the Texas Attorney General’s Consumer Protection Division.
